Through partnership with Triad Coordinated Services, Inc. clients will have an Individual Service Plan (ISP) that is tailored to their desires, abilities and needs as part of their habilitative services. We strongly encourage clients and their family's to participate in the design and delivery of their Individual Service Plan. The ISP outlines the frequency of your publicly funded service(s) and no additional fees are required for these services. The ISP is utilized to help each person achieve his or her individual goals. Goals can range depending on the client, but usually include areas of self-help, socialization, communication, independent living, education, and community inclusion.

 

Each client has a unique care team that comes together in helping them achieve their goals in the home and in the community. All TCS' services are provided during times and days centered on the needs of each person served while maintaining compliance with each service definition. The team actively helps the client develop skills while also helping them to explore options for available resources, referrals, supports, and assistance. TCS believes in the importance of team members listening, learning, reflecting, and taking action so that a client's full potential may be realized.
