Your Role...

Job Description:

  • Implement individualized goals according to the treatment plan. 

  • Empower individuals to live successfully within their homes.

  • Enhance individuals' opportunities to have full membership within their communities.

  • Submit clinical documentation reflecting billable time in a timely manner.

  • Follow all policies and procedures of Triad Coordinated Services, Inc. (including, but not limited to client rights, confidentiality of client information, service provision, and documentation).

 

Qualifications: 

  • High school diploma or GED required. 

  • Must be at least 18 years of age.

  • Must have a valid driver's license, safe driving record, and acceptable level of auto liability insurance.

  • Experience in working with children or adults with intellectual and developmental disabilities, autism, or mental illness is preferred, but not required. 

  • Have an acceptable Healthcare Registry check, Criminal Record check, and ethical dealings with federal/state government check.
